    Clinical review of pediatric pilocytic astrocytomas treated at a tertiary care hospital in Pakistan

    Abstract Background: Pilocytic Astrocytoma (PA) is a common type of brain tumor in the pediatric population. They have a fairly good prognosis. This study describes PAs in detail, with a focus on the demographic factors, presenting features, management and prognosis, and aims, to identify the negative outcome predictors in our population, which can affect the course of the disease. This article will add to the understanding of PAs from a third world perspective. Methods: The Aga Khan University medical records (1995 – 2007) were reviewed, to study the clinical features, management, and outcome of patients (0 – 15 years) with Pilocytic Astrocytomas (PAs) in our population. After a thorough review of the medical records, all the PAs diagnosed on the basis of histopathology at our Pathology Laboratory, during this period, were included in the study. Results: Twenty-two patients were included with a mean age of 9.25 years. Male-to-female ratio was 1 : 1. The most common presenting feature was a sign of increased intracranial pressure. The most common location was the cerebellum followed by the cerebrum. Fifteen patients underwent maximum surgical resection. Three had recurrence, despite no residual tumor. There were 10 Intensive Care Unit (ICU) admissions and one inpatient mortality. Fifteen patients followed up in the clinic: Eight had recurrence and four underwent repeat surgery (three showed clinical improvement). Hydrocephalus was a predictor of ICU admission. Solid consistency was found to be a marker of recurrence. Conclusion: Pilocytic Astrocytomas are the most common pediatric brain tumors in our population, commonly located in the cerebellum. Complete resection is the best treatment option, but some tumors are aggressive and recurrence is not uncommon. The possible negative outcome predictors are age, source of admission, extent of resection, hydrocephalus, and solid consistency

    A Novel Derivatization Ultraviolet Spectrophotometric Method for the Determination of Amlodipine Besylate Using Benzoyl Chloride

    The present research work aims to develop a novel ultraviolet UV spectrophotometric method for the determination of Amlodipine Besylate using Benzoyl Chloride as a derivatizing agent, which is simple, rapid, sensitive, selective, and accurate method for the spectrophotometric determination of Amlodipine Besylate in powder form.  Synthesis is based upon the Schotten Baumann Reaction. In this method, derivatization of aliphatic amine group of Amlodipine Besylate carried out with benzoyl chloride and aqueous sodium hydroxide (NaOH).The λmax was found to be 237 and 226nm for assay of Amlodipine Besylate and synthesised product respectively. The linearity was found in concentration range of 1-10 μg/ml. The correlation coefficient (r2)was found 0.9985. The regression equation, intercept (a) and slope (b) was found as Y=0.0762x - 0.0077, 0.0077 and 0.0762 respectively. Method was developed and validated as per ICH guidelines for linearity, accuracy, precision, LOD, LOQ, interday and intraday. The LOD and LOQ for estimation of Amlodipine besylate were found as 0.2367, 0.7178 respectively. Recovery of Amlodipine besylate was found to be 93.30%.The proposed method is found to be simple, rapid, selective and highly sensitive than most of the Spectrophotometric methods available in literature.     West Nile Virus (WNV) is a pathogenic arbovirus that belongs to genus Flavivirus under family Flaviviridae. Till now there are no approved vaccines against WNV for human use. In this study, the effect of two alkylating agents, formaldehyde and β-PL, generally used for inactivated vaccine preparation, was assessed on the basis of antigenic and immunogenic potential of the inactivated WNV. Lineage 5 WNV isolates were inactivated by both formalin and β-PL treatments. Inactivation was confirmed by repeated passage in BHK-21 cell line and infant mice. Viruses inactivated by both the treatments showed higher antigenicity. Immune response in mice model showed serum anti-WNV antibody titre was moderately higher in formalin inactivated antigen compared to β-PL inactivated antigen. However, no significant differences were observed in neutralization antibody titre. In conclusion, we can state that both formaldehyde and β-PL inactivation processes were found to be equally efficient for inactivation of WNV. However, they need to be compared with other inactivating agents along with study on cell mediated immune response

    Perceptions of 'coming out' among British Muslim gay men

    The cultural processes of heteronormativity and compulsory heterosexuality are acutely active within Islamic societies. The present study explored perceptions of 'coming out' among a group of British Muslim gay men (BMGM), focussing upon the potential consequences for identity processes and psychological well-being. Ten BMGM of Pakistani descent were interviewed using a semi-structured interview schedule. Interview transcripts were subjected to interpretative phenomenological analysis and informed by identity process theory. Four superordinate themes are reported, including (1) 'social representational constraints upon "coming out"'; (2) ' "coming out": a source of shame and a threat to distinctiveness'; (3) 'fear of physical violence from ingroup members'; and (4) 'foreseeing the future: "coming out" as a coping strategy'. Data suggest that BMGM face a bi-dimensional homophobia from ethno-religious ingroup members and the general population, which can render the prospect of 'coming out' threatening for identity. Theoretical and practical implications of this research are discussed

    Dengue Outbreak in a Hilly State of Arunachal Pradesh in Northeast India

    outbreak of fever with unknown origin (FUO) indicative of Dengue was reported in Pasighat, East Siang district of Arunachal Pradesh (AP) state. Serum samples (n = 164) collected from patients from Health Training and Research Centre General Hospital, Pasighat, were tested for NS1 antigen and IgM antibodies. NS1-positive samples were analyzed by RT-PCR assay and entomological surveys were carried out. The majority of suspected cases reported NS1 antigen positivity. Females and young adults were mostly affected. The majority of the amplified NS1-positive samples showed Dengue serotype 3 infection. Aedes (Stegomyia) albopictus, known as semiurban breeding mosquitoes, was the only potential vector species identified from the affected areas of Pasighat which single handedly contributed to the outbreak. Thus, the present work identifies Dengue as an emerging arboviral infection in hilly state of AP along with a looming risk of its spread to neighbouring areas

    Numerical Study on Bubble Dynamics and Two-Phase Frictional Pressure Drop of Slug Flow Regime in Adiabatic T-junction Square Microchannel

    In this study, bubble dynamics and frictional pressure drop associated with gas liquid two-phase slug flow regime in adiabatic T-junction square microchannel has been investigated using CFD. A comprehensive study on the mechanism of bubble formation via squeezing and shearing regime is performed. The randomness and recirculation profiles observed in the squeezing regime are significantly higher as compared to the shearing regime during formation of the slug. Further, effects of increasing gas velocity on bubble length are obtained at fixed liquid velocities and simulated data displayed good agreement with available correlations in literature. The frictional pressure drop for slug flow regime from simulations are also obtained and evaluated against existing separated flow models. A regression correlation has also been developed by modifying C-parameter using separated flow model, which improves the prediction of two-phase frictional pressure drop data within slug flow region, with mean absolute error of 10 %. The influences of fluid properties such as liquid viscosity and surface tension on the two-phase frictional pressure drop are also investigated and compared with developed correlation. The higher liquid viscosity and lower surface tension value resulted in bubble formation via shearing regime. This work is licensed under a Creative Commons Attribution 4.0 International License
